Submit Application, Oath, Exam

After purchasing your notary package, please submit the following by either Email or Mail to the Wisconsin Secretary of State:

Your signed notary application.
Your signed Oath of Office form
Your certificate from passing the Notary Exam with 90% or better (taken within 1 year of the date of submission of the application)
Your notary bond information
